Artrocentesis: Articulacion Temporo - Mandibular (Obtenlo gratis)

Introducción
La artrocentesis en la articulación temporomandibular (ATM) es el procedimiento de aspiración de líquidos en el espacio articular superior por medio de una o varias agujas y la inyección
posterior de una sustancia terapéutica bajo presión hidráulica.1-3 En nuestro país, los Aztecas la utilizaron en articulaciones inflamadas, Fray Bernardino de Sahagún relata que ellos fueron una de las primeras culturas en realizar estos procedimientos usando un tipo de espina poco conocida. Describieron la naturaleza de los líquidos aspirados comparando su viscosidad con la de la baba del nopal.4

Los primeros usos informados en la literatura médica incluyen el alivio sintomático del dolor por sinovitis traumática y como un auxiliar para el diagnóstico y manejo terapéutico de hemartrosis, artritis, seudogota, lupus eritematoso sistémico (LES) y artritis séptica. Anatómicamente ha sido utilizada en las articulaciones de la cadera, rodilla, tobillo, manubrio del esternón, tarso, carpo y sus falanges. 1,2 La artrocentesis es considerada como un procedimiento no quirúrgico, simple,
eficiente y con riesgo mínimo, que puede ser realizado bajo anestesia local con o sin sedación, en condiciones estériles y con mínimas complicaciones como contaminación, daño a algunas ramas nerviosas e irritación local por introducción de cuerpos extraños.

REUMATOLOGIA: Estandares de Calidad Asistencia (Obtenlo gratis)

Objetivo: Elaborar estandares de calidad asistencial y de tiempos de los procesos reumatologicos.
Material y metodos: Tras una revision sistematica de la literatura un grupo de trabajo, onstituido por 10 reumatologos y 2 metodologos, ha seleccionado 164 indicadores de calidad y de tiempos de proceso.
Un panel de 65 expertos los ha puntuado mediante mctodologia Delphi.
Resultados: Entrc los 164 estandares obtenidos destacan:
numero maximo de habitantes por reumatologo (40.000 - 50.000); numero minimo de reumatologos en una unidad de reumatologia (3); tiempo de duracion de la primera consulta (30 min) y la sucesiva (19 min), relacion sucesiva/primera consulta (3,2); un reumatologo no debe
atender mas de 5 primeras visitas/dia y no mas de 11 consultas sucesivas/dia, no debe pasar consulta mas de 5 h/dia, ni mas de 4 dias/semana; el tiempo maximo de espera para la primera consulta no sera superior a 4 semanas; el tiempo necesario para la visita de pacientes ingresados (43 min al ingreso, 17 min la visita diaria y 40 min el alta); tiempo para la realizacion de las tecnicas mas frecuentes en reumatologia (12 min para artrocentesis e infiltraciones y 17 min para analisis del liquido sinovial).
