REVASCULARIZACION CORONARIA: tratamiento

PUNTOS CLAVE
Tratamiento con revascularización. La estenosis de las arterias coronarias puede ser tratada con
técnicas de revascularización percutánea (angioplastia) o a través de cirugía de by-pass.
Cateterismo cardíaco. Permite el registro de la presión dentro de las cavidades cardíacas, el
estudio de las valvulopatías, de las anomalías estructurales y de las arterias coronarias gracias

Angioplastia coronaria. Consiste en dilatar los segmentos estenóticos de las arterias coronarias mediante técnicas percutáneas y aliviar de esta manera la sintomatología de pacientes que padecen de angina de pecho o de infarto agudo de miocardio

• La angioplastia inicialmente se llevó a cabo con balón, pero tenía limitaciones: alto porcentaje de reestenosis tras el tratamiento inicial.
Stents. La angioplastia con stent, asociada al régimen terapéutico de doble antiagregación con
ácido acetilsalicílico y clopidogrel, ha disminuido sustancialmente las tasas de reestenosis

• Actualmente la gran mayoría de las angioplastias terminan con implantación de stent

• La principal indicación de angioplastia con implantación de stent en la angina estable sería
el control de los síntomas si éstos son persistentes a pesar de la medicación antianginosa

• La aparición de stents farmacoactivos ha reducido aún más las tasas de reestenosis intrastent, y ha hecho posible el tratamiento percutáneo de lesiones cada vez más complejas.


Cirugía de revascularización. Permite implantar injertos venosos o arteriales en las arterias
coronarias enfermas, asegurando la irrigación del músculo cardíaco distal a las estenosis coronarias

• Las indicaciones principales de cirugía de revascularización son la enfermedad
de tronco común izquierdo y la enfermedad de tres vasos.

MANEJO GENERAL Y EXTRAHOSPITALARIO DEL ANGOR PECTORIS


Introducción
La angina de pecho es el síntoma más característico de la cardiopatía isquémica (CI) y es manifestación de isquemia miocárdica. La Sociedad Española de Cardiología la definió como “dolor, opresión o malestar, generalmente torácico, atribuible a isquemia miocárdica transitoria”. Esta definición permanece vigente en nuestro medio. La angina estable es, por definición, aquella angina de esfuerzo en la que no ha habido cambios en su patrón de presentación en el último mes de evolución 1-3.
Características del dolor torácico

1. Dolor opresivo retroesternal con irradiación típica (brazos, cuello, etc.).
2. Provocado por el esfuerzo o la tensión emocional.
3. Se alivia con reposo o nitroglicerina.

En función de estas características se clasifica:
a) angina típica que cumple las tres características;
b) angina atípica que cumple dos características; y
c) dolor inespecífico o no coronario que cumple una o ninguna característica.

El diagnóstico de presunción de angina es de probabilidad, por lo que para hacerlo se debe tener en cuenta, además de las características del dolor, la presencia de otros factores de riesgo que aumentan de forma importante la probabilidad de que el dolor sea isquémico: antecedentes familiares de CI o muerte súbita precoces, sexo (entre los jóvenes riesgo mucho mayor en varones), diabetes como el factor de mayor riesgo para CI, tabaquismo, hipertensión arterial (HTA), dislipemia y enfermedad vascular previa cerebral (accidente isquémico transitorio/accidente vascular cerebral) o periférica (claudicación intermitente).

ANGINA ESTABLE: Concepto - Diagnóstico - Pronóstico (Actualización)

CONCEPTO
La angina estable es un síndrome clínico debido a isquemia miocárdica, caracterizado por malestar en el pecho, las mandíbulas, los hombros, la espalda o los brazos, que aparece con el ejercicio o estrés emocional y remite con el reposo o con la administración de nitroglicerina. Aunque la causa más común de la isquemia miocárdica es la aterosclerosis coronaria, existen
otros procesos cardiovasculares que la causan, como la miocardiopatía hipertrófica o dilatada o la estenosis aórtica 1.


EPIDEMIOLOGIA


La prevalencia de la angina aumenta marcadamente con la edad en ambos sexos, de un 0,1-1% en mujeres de edades comprendidas entre 45 y 54 años a un 10-15% en las de 65- 74 años, y de un 2-5% en varones de 45-54 años a un 10 - 20% en los de 65-74 años. Se calcula que en la mayoría de los países europeos entre 20.000 y 40.000 individuos por millón de habitantes sufren angina 2.



HISTORIA NATURAL
La información sobre el pronóstico relacionado con la angina crónica estable se ha obtenido de estudios poblacionales prospectivos a largo plazo, de ensayos clínicos sobre terapias Antianginosas y de registros observacionales, en los que la selección de pacientes produce un sesgo importante que hay que tener en cuenta al evaluar y comparar los datos disponibles. Datos recientes recogidos en ensayos clínicos sobre terapia antianginosa y/o revascularización indican que la tasa anual de mortalidad varía entre el 0,9 y el 1,4% por año, con una incidencia anual
de infarto de miocardio no fatal de entre el 0,5 y el 2,6%1.

Therefore, when treated with a drug, the sensitive cells in a tumor succumb to the toxic effects of the drug, and resistant cells continue to survive and multiply. The tumor, which re-grows from these residual resistant cells, is not sensitive to the original drug.

ESTATINAS E ICTUS ¿Qué hay de nuevo?

El tratamiento utilizado para reducir los niveles elevados de colesterol podría ser una buena opción terapéutica para los ictus
Las estatinas, fármacos que reducen el nivel de colesterol previniendo así enfermedades cardiovasculares, también podrían servir para el tratamiento de la fase aguda de los accidentes vasculares cerebrales. Cada vez con más evidencias científicas que apuntan en esa dirección y ya superada la fase experimental, se empiezan a aplicar en muchos centros hospitalarios. No obstante, aún no están incorporadas como indicación formal en las guías de práctica clínica y, por lo tanto, no es un tratamiento habitual. ¿Será el futuro en el ictus?

El colesterol, ese temible enemigo

El colesterol, temido por buena parte de la población adulta, es un conocido enemigo del corazón. Pero también puede serlo del cerebro. En esta segunda faceta es menos conocido e importante que otros factores de riesgo responsables de los infartos cerebrales, pero lo cierto es que también puede conducir a un ictus o accidente cerebrovascular (ACV). Así, la hipertensión arterial es un factor de riesgo más potente que el colesterol para provocar un ictus, mientras que el colesterol lo es para originar un infarto agudo de miocardio.
Sin embargo, esta realidad no puede hacer olvidar que el colesterol también puede favorecer un ictus, según ha explicado Joan Montaner, director del Laboratorio de Investigación Neurovascular del Servicio de Neurología del Hospital del Valle de Hebrón de Barcelona. «El impacto del colesterol sobre los ictus es relevante, pero no tan importante como en el corazón. El cerebro sufre más por la hipertensión arterial, mientras que el colesterol tiene un impacto mucho menor», explica Jaume Roquer, jefe del Servicio de Neurología del Hospital del Mar de Barcelona.
Al corazón le sucede al revés: el colesterol es más lesivo y la hipertensión arterial no tanto. «Pero cabe decir que el impacto del colesterol está muy discutido en patología cerebrovascular», ha matizado Roquer. Dentro de los distintos tipos de ictus, el colesterol está más relacionado con los ictus ateromatosos, que son aquellos que ocurren por oclusiones de distintas arterias, bien porque se produce una estenosis (o estrechamiento) de la arteria carótida (los vasos del cuello) o de las intracraneales (en el cerebro). En total, es el responsable del 25% de los ictus. «Este no es un porcentaje desdeñable, aunque en otras sociedades aún es más importante», comenta Montaner.


Papel de las estatinas


Hasta ahora, las estatinas se han utilizado para reducir los niveles elevados de colesterol, puesto que, en exceso, éste es un importante factor de riesgo cardiovascular. La primera estatina, simvastatina, apareció en el mercado en 1984 y, desde entonces, sus beneficios para prevenir la enfermedad coronaria han quedado bien demostrados. Se emplean, sobre todo, en las personas que tienen un exceso de colesterol y placas de ateroesclerosis que entrañan riesgo de cardiopatías.
Su papel principal ha consistido en la prevención primaria de los infartos de corazón (evitarlos cuando aún no se han producido). Según Montaner, de momento ningún estudio ha demostrado que las estatinas sean útiles en la prevención primaria del ictus, es decir, para evitar un primer ictus. En cambio, estudios realizados por su grupo empiezan a demostrar que la administración de estatinas puede ser útil tras uno de estos episodios, como prevención secundaria, así como para mejorar la evolución y complicaciones de los infartos cerebrales.
En concreto, las estatinas se aplican en la fase aguda del ictus, es decir, durante las primeras horas que siguen al accidente cerebrovascular. Además, la administración de estatinas no tendría como objetivo disminuir el colesterol, sino estabilizar la placa de ateroma y así favorecer la recuperación de las neuronas que mueren durante la fase aguda de estos episodios. ¿Por qué? Porque las estatinas inhiben una vía que también es responsable de otros efectos, llamados pleiotrópicos, la mayoría de los cuales están relacionados con fenómenos de inflamación.
Las estatinas son antiinflamatorios muy potentes que evitan que progrese la ateromatosis en la fase aguda del ictus, donde hay una inflamación muy importante. «En los infartos cerebrales hay unas células de la serie blanca, neutrófilos o morfonucleares, que pueden causar mucho daño. Mientras que las estatinas, administradas en esta fase aguda, actuarían como antiinflamatorios y neuroprotectores», precisa Montaner sobre el papel de estos fármacos.

Acción y efecto de reanimar. Conjunto de medidas terapéuticas destinadas a la recuperación y mantenimiento de las constantes vitales del organismo, que se aplican en condiciones en que éstas se encuentran comprometidas.

Reanimación cardiopulmonar (CPR) es la aplicación de tratamientos farmacológicos y mecánicos encaminados a preservar la función celular de órganos vitales y producir la reanudación de la circulación espontánea.
Comenzó con el masaje cardíaco externo en 1960 (se aplicó por 1ª vez en un niño de 5 años en 1958).

La reanimación del recién nacido no se define sólo por un cuadro, por un algoritmo en el que se hace referencia al empleo de gases, fármacos y a la aplicación de métodos físicos o mecánicos para la iniciación y/o mantenimiento de la respiración en el momento del nacimiento, ya que ello implicaría una consideración restringida y limitada del problema.


El concepto que propugnamos y defendemos es el total: abarca la investigación, el diagnóstico precoz, la valoración de todas las influencias que operan antes de la concepción, durante el embarazo y alrededor del nacimiento que nos permitan el uso de medidas preventivas o la aplicación inmediata de procedimientos bien concebidos para reanimar al niño en caso de fracaso o fallo. Este amplio punto de vista es el que constituye el principio básico de la reanimación de los recién nacidos humanos.
El R.N. tiene una historia anterior, reflejará, por tanto, la suma total de su pasado genético y de sus circunstancias ambientales.

LEUCEMIA AGUDA (Nuevos tratamientos: bajarlos gratis)

LEUCEMIA AGUDA
Son neoplasias que comprometen el sistema hematopoyético (sector linfoide o mieloide) con infiltración de elementos blásticos en médula ósea que a través de la sangre periférica se disemina al resto del organismo. Constituyen el 35% de las enfermedades malignas en la infancia.
Pueden diferenciarse las leucemias en agudas y crónicas, de acuerdo a la presencia
predominante o no, de precursores inmaduros, hematológicos o linfoides.



Las congénitas son las diagnosticadas en las primeras cuatro semanas de vida. De acuerdo al predominio de la línea celular involucrada se clasifican en linfoblásticas y no linfoblásticas (mieloblásticas).
El 80% de las leucemias en Pediatría son linfoblásticas, el 15% mieloblásticas y el 5% indiferenciadas.
La caracterización de los blastos se hace por métodos morfológicos, citoquímicos, marcadores inmunológicos de membrana, marcadores citoplasmáticos y citogenéticos.
De ello dependerá el criterio pronóstico y de tratamiento.

La clasificación morfológica standarizada internacionalmente para la Leucemia Linfocitica Aguda (LLA) es L1, L2 y L3 y para la leucemia Mielocitica Aguda (LMA) es M0 a M7. Lo más común
en la infancia es L1.
Criterios de internamiento
Estudios diagnósticos.
Prevención de complicaciones en pacientes de riesgo: neutropenia febril y trombocitopenia sintomática.
Tratamiento de complicaciones: infección y granulocitopenia, hemorragia, anemia, trastornos metabólicos, neurológicos, obstructivos de las vías aereas, hiperleucocitosis.
Con frecuencia, la leucemia aguda en adultos puede curarse.
Los tratamientos pueden incluir quimioterapia, radioterapia y trasplantes de células madre. Aún si los síntomas desaparecen, usted podría necesitar tratamiento para prevenir una recaída.

DIABETES MELLITUS:

Es una de las enfermedades mejor y más conocidas de la historia de la medicina. Aunque el nombre científico de la enfermedad, diabetes mellitus significa “miel”, no ha sido hasta el siglo pasado cuando se confirmó que la elevación de la glucosa en sangre es la principal característica de esta enfermedad. Gracias al importante aumento en el conocimiento de la diabetes se ha demostrado que no es una afectación única, sino un síndrome en el que hay una alteración de múltiples órganos; y por lo tanto presenta diferentes entidades nosológicas. La causa común de todas ellas es el aumento de los niveles de glucosa en sangre o hiperglucemia. Esta hiperglucemia se debe a un déficit absoluto o relativo de la acción de la insulina, lo que produce en términos generales un síndrome metabólico y un síndrome vascular.

El síndrome metabólico produce a parte del aumento de glucosa en sangre una alteración en el metabolismo de las proteínas y de los lípidos, siendo más frecuente el padecimiento de arterioesclerosis en estos pacientes que en la población general .

El síndrome vascular resulta de la lesión producida en los pequeños vasos del organismo por la hiperglucemia ( microangiopatía ); lo que conlleva la aparición de complicaciones renales denominadas nefropatía, complicaciones a nivel de la retina o retinopatía, y a nivel de los nervios o neuropatía diabética. También se produce una afectación de los grandes vasos denominada macroangiopatía que produce problemas principalmente a nivel cardiovascular. Estas complicaciones microvasculares y cardiovasculares aumentan sustancialmente la mortalidad y morbilidad asociada a la enfermedad, y reducen la calidad de vida.
